Assessment Forms – What to Use and When
1. Which form do I use?
Depends on your training pathway:
From 2026 PGY1 & PGY2 (AMC Framework)

Complete mid-term and end-of-term assessments on the CLA Portal (Clinical Learning Australia).
PGY2+ in a college training program (e.g. BPT, Anaesthetics)

Use your college’s assessment forms, OR the standard Northern Doctors Education (NDE) editable PDF forms above.
PGY3+ (not in a training program)

Forms are optional, but recommended for those applying to specialty programs (e.g. GP Training).
IMGs
-
Requirements differ (see IMG section below).
2. If using the NDE Assessment Form

The same PDF is used for both mid-term and end-of-term—just tick the correct box.

Supervisor should complete it digitally, then email to NDE.
-
Paper/scanned forms are accepted but not encouraged.
-
Keep a copy for your own records.
3. Mid-Term Assessment
Purpose
-
Receive timely performance feedback
-
Identify emerging learning needs
-
Plan how to address these for the remainder of the term
Who can complete it?
-
Registrar or fellow as day-to-day supervisor
-
Must be countersigned by a consultant
-
If unsure or have concerns → contact NDE.
Tips
-
Arrange within the first 4 weeks, as consultant rosters change monthly.

End-of-term and mid-term use the same form.
4. End-of-Term Assessment
Purpose
-
Feedback on performance
-
Feedback on the unit (training, workload, culture)
-
Discuss career direction
-
Quick wellbeing check
Process
-
Contact your term supervisor 2–3 weeks before term ends.

Editable PDF preferred.
-
Email completed form to NDE.
-
Retain a personal copy for future training applications.

Share learning goals with your next term supervisor for continuity.

5. Interns – Special Notes
Rotation 1 (Term 1)

Complete your mid-term assessment in week 3 with the current registrar (registrars change in week 4).

It is appropriate for a registrar who has supervised you for only a few shifts to complete it.
6. IMGs (International Medical Graduates)
WRIG30 Requirements
-
Continue completing WRIG30 with your AHPRA-allocated Principal Supervisor.

Email completed WRIG30 to Northern Doctors Workforce (NDW) before submitting to AHPRA.

If you’re unsure who your principal supervisor is, contact Kai Cooper, IMG Compliance Officer at NDW.

If you don’t work regularly with them, ask your term supervisor to complete the IMG Assessment Form above for mid/end-term feedback.
New IMGs
-
Submit a WRIG30 every term, with a mandatory one at 3 months.
-
After 12 months, submit all WRIG30s to AHPRA for renewal or application for general registration.
Once you obtain general registration
-
Feedback becomes optional but still helpful, especially for future college program applications.
7. PGY2 in College Programs & PGY3+
-
Can participate in the AMC Framework assessments but not mandatory.
8. Special Situations
Short rotations (e.g. Broadmeadows Med Nights, specialty nights)
-
End-of-term only (no mid-term required).
Split surgical terms (e.g. AGSU/HPB)
-
Submit an End-of-Term Assessment for each split.
9. Who Can Complete Assessments
Term Supervisor
-
Provides overall supervision, education, assessment, and wellbeing support.
-
May or may not be your primary clinical supervisor.
Primary Clinical Supervisor
-
Consultant/senior doctor responsible for patient management during the term.
-
May change throughout the rotation.
Registrar (PGY3+)
-
Day-to-day clinical supervisor.
-
Can complete mid-term; consultant must countersign.
Assessment Review Panel (ARP)
The panel monitors each PGY1 and PGY2 doctor’s progress across the year to ensure they are meeting required learning outcomes and have the support needed to advance. At year’s end, the panel makes an overall judgement on whether the doctor is ready to progress to the next stage of training. Both mid and end of term assessments are required for each term for the panel to make a decision.
