Closing date for Registrar applications at Broadmeadows Health Service is 5.00pm on Thursday 16th August 2007.

1. Three (3) Referee Assessments are required. At least two (2) should be from Consultants. Registrars possible for one (1) only.

2. Complete the Referee form in full, prior to forwarding to your Referee, to ensure that this assessment is successfully matched to the job(s) you are applying for.

3. As your Referee is to send the completed Form directly to the Hospitals, tick on the back page, the Health Service to whom you are applying and to whom the assessment is to reach.

4. Referees may also send their forms directly to TNH via clinicalsupport@nh.org.au
