1. Three (3) Referee Assessments are required from Consultants. One may be from your current Registrar.
2. Complete the "Application Details" section including "Position Applied for".
3. On the reverse side of the form tick the Health Service(s) to whom you wish to apply.
4. Referees may fax or send their forms directly to TNH via mail.

JOINT HMO3 POSITIONS - Austin and Northern Health
A variety of jointly recruited HMO3 positions are available in a number of disciplines. Please also see Austin Health advertisement for information regarding jointly appointed HMO 3 & 4 positions at www.austindoctors.org.au.
International Medical graduates are also required to apply for registration with the 'Medical Practitioners Board of Victoria' and to obtain the appropriate work visa.
